Is my text sent to a server?

No. Speech is produced by the synthesiser built into your operating system through the browser Speech Synthesis API, so the text is spoken locally and never uploaded.

Why do I only see a few voices?

The list comes from your device, not from this site. Windows, macOS, Android and iOS each ship a different set, and most systems let you install extra language packs, which then appear here after a reload.

Can I download the audio as an MP3?

Not from the browser speech API — it plays audio but does not expose a file. If you need a file, record the system audio output, or use a service that renders speech server-side.

Does it read other languages?

Yes, provided a voice for that language is installed. Pick the matching voice, since an English voice reading Hindi or Spanish text will mispronounce almost every word.
